What is the most frequent cause of dry eye amongst patients you have treated?

The most frequent cause(s) of aqueous-deficient dry eye among patients I treat is chronic inflammation and waning testosterone levels in peri-menopausal women. Chronic blepharitis/meibomian gland dysfunction is often overlooked as a culprit. It is the primary cause of evaporative dry eye.

What advice do you most frequently find yourself giving to dry eye patients?

The correct diagnosis of the fundamental type of dry eye is of greatest importance. It is vital that patients realize that dry eye conditions are chronic and that successful control of the condition is very possible with patience and compliance.

Do you have a dry eye "hot button"?

Both the DEWS Report and the Delphi Panel give good guidelines for rational treatment of an aqueous-deficient dry eye. Without doubt, blepharitis/meibomian gland disease is the most overlooked condition leading to dry eye problems and is often confused with a dry eye caused by aqueous deficiency. The treatment approach for each is completely different. Again, everything is subordinate to accurate diagnosis.

What makes you exceptionally well qualified to help dry eye patients?

I like to think that first and foremost I really listen to my patients. A great deal of compassion and patience are attributes I strive to express. I believe I am very strong in diagnosis with the help of extensive testing and examination. I like to create sensible and rational treatment plans, individually tailored, that are effective in minimizing the suffering of chronic dry eye. I truly want my patients to enjoy more freedom from the discomfort, distraction, and inconvenience of their condition so they can lead more productive lives.

What are some of your favorite products for dry eye (Rx, OTC, consumer)?

There are many excellent Rx and OTC products having specific uses depending on the type of dry eye being treated. A partial list of products I use regularly includes: Genteal Gel, Systane Ultra, preservative-free Systane, Systane ointment, Optive, Blink, Freshkote, Soothe, Restasis, Refresh Endura, Refresh Plus, Refresh Tears, Celluvisc, Oasis Tears and Tears Plus, Biotears nutritional, a host of other nutritional products and Oasis Formfit punctal plugs.
