What is the most frequent cause of dry eye amongst patients you have treated?

Elderly patients who have untreated lid disease.

What advice do you most frequently find yourself giving to dry eye patients?

Dry eyes are a common problem and often require more than one type of treatment. The first line of treatment is an artificial tear that is prescribed by the eye doctor to be used daily as directed by the doctor. Many times a supplement of Omega 3 is used in conjunction with the artificial tear.

Do you have a dry eye "hot button"?

Both blepharitis and meibomianitis are often overlooked or not treated properly. Warm compresses with lid scrubs when performed correctly can be very effective and relieve many symptoms that the patient is having.

What makes you exceptionally well qualified to help dry eye patients?

I care about my patients. I spend time to explain their condition and how to treat it. We schedule follow-up appointments to make sure that the patient is making progress. The treatment often times needs to be updated.

What are some of your favorite products for dry eye (Rx, OTC, consumer)?

1. My favorite artificial tears are Systane and Optive.

2. Favorite Rx products are Restasis and Lotemax.

3. Antioxidant: Omega 3


Short Bio - Dr. Carl Garbus

Dr. Carl Garbus graduated from the Southern California College of Optometry in 1980. His practices emphasize care for children and adults with vision deficits from traumatic brain injury, stroke and corneal problems such as keratoconus and corneal transplants. Specialty contact lenses for irregular corneas and dry eyes are fit by Dr. Garbus including scleral lenses.

Dr. Garbus has hospital privileges and consults with Cedars Sinai Medical Center, Providence Holy Cross Hospital, Good Samaritan Hospital and Henry Mayo Hospital. In addition, Dr. Garbus works with the following rehabilitation ceners: Centre for Neuro Skills, Solutions At Santa Barbara and The Rehabilitation Institute of Santa Barbara. He has made various presentations and in-services to occupational therapy groups, as well as PTA groups regarding vision problems. Another area of interest is working with children who have learning problems. His new office is equipped to help children who have autism.

Dr. Garbus was president of the Los Angeles County Optometric Society and currently serves on the board for the Neuro Optometric Rehabilitation Association. He is in practice with Dr. Lori Nishida and Dr. Hetty Kim. Their office goes under the name Family Vision Cae Optometry.

For dry eye patients here are the pros and cons to fitting scleral and miniscleral lenses:

PROS

1. more comfortable

2. faster adaptation

3. less tearing

4. less problems with dryness

5. larger optical zones (quality of vision may be improved)

6. less chance of external debris collecting under the lens

7. allows for les centration on a highly irregular cornea

8. more profit, if successful

CONS

1. more difficult to fit = more chair time

2. less lens movement

3. less tear circulation under the lens

4. harder to apply and remove

5. can tighten up during the wearing

6. may have to use a DMV remover

7. requires close supervision

8. traps epithelial cell debris

When to consider fitting a scleral lens or miniscleral:

1. Gas permeable lenses are uncomfortable and ride low

2. Soft lenses are uncomfortable and dry out easily

3. Corneal transplants that have large amounts of irregular astigmatism
